If you’ve been looking into wrinkle treatments in Seoul, you’ve probably seen Botox and filler mentioned almost interchangeably. They’re both popular, both injectable, and both take about 15–30 minutes. But they work in completely different ways. And choosing the wrong one can leave results looking less natural than they should.

At Forena Clinic in Hongdae, we see this question almost every day. Here’s a clear, practical breakdown so you can understand what each treatment actually does.

Two Types of Wrinkles

Most people have a mix of both, and they need different approaches.

Dynamic wrinkles appear when you smile, frown, or raise your brows, then soften or disappear when your face is at rest. They’re caused by repeated muscle movement folding the skin in the same place over time. Common areas include the forehead, the lines between the brows (glabella), and crow’s feet.

Static wrinkles stay visible even when your face is completely relaxed. These come from lost volume or support under the skin, or from a crease that has set in over the years. You’ll often notice them as nasolabial folds, marionette lines, or deeper lines around the mouth and cheeks.

How Botox Works for Dynamic Lines

Botox (botulinum toxin) relaxes the muscle so it folds the skin less. Softening usually starts in 3–7 days and typically lasts 3–4 months. Downtime is minimal. Small injection marks usually settle within hours.

It’s the go-to option for expression lines because it treats the cause: the movement itself. At Forena we prefer conservative doses so natural movement stays.

How Filler and Collagen Boosters Work for Static Lines

Fillers add volume or support under the skin. Results are visible right away and typically last 6–18 months depending on the product and area. Collagen boosters work more gradually, encouraging your skin to rebuild support over time. Mild swelling can last a few days.

These treatments address what toxin cannot: lost volume or a crease that remains at rest.

Matching the Concern to the Treatment

  • Crow’s feet or forehead lines that only show when you smile or raise your brows → Botox in small, conservative doses

  • Nasolabial folds or marionette lines that stay when your face is relaxed → Filler or a collagen stimulator

  • Fine, etched surface lines with dull or dry texture → Skin booster, micro-toxin, or light resurfacing

What Happens When the Wrong Treatment Is Used

Filler placed in a purely dynamic wrinkle can look less natural because the muscle keeps folding the skin over it.

Botox alone on a static fold can soften movement around the area, but it cannot replace lost volume or support.

When the treatment matches the actual cause of the line, results tend to look more natural and last longer.

How We Decide at Forena

  1. We look at the face both at rest and with expression.

  2. We treat dynamic lines with toxin, usually at conservative doses so natural movement stays.

  3. If static lines remain, we add filler, a collagen booster, or lifting as needed.

  4. We plan each area separately according to its cause.

Many patients need both. We usually start with toxin for the dynamic component, then assess what’s left. Treating only one sometimes makes the other more noticeable.

What to Expect

  • Toxin wears off after 3–4 months and filler after 6–18 months, so top-ups are part of the plan.

  • We aim for soft results that still let your face move. This is the look most patients in Korea prefer.

  • Results look unnatural most often for two reasons: too much filler in movement-related lines, or too little treatment for lines that stay at rest.

  • A consultation is always required first so we can check skin quality, volume loss, and muscle strength before recommending anything.
